In addition to creating issues from the Issues tab, you can also create them wherever you create a fact.

You can create new issue tags in two places on the facts page in your case:
At the top-right of your main facts list, click the chevron next to the "Create Fact" button. This will reveal a drop-down menu where you can select "Create Issue":
You can also create an issue when editing an individual fact record:
Creating Issues in the Document Reviewer
Similarly, you can create issues from the document reviewer. Click the "Cite Page" button or highlight text to open the Create Fact sidebar, then click the "+ Create a new issue" link under the Issues field:
